Job Overview

We are seeking a dedicated and experienced Advanced Practice Registered Nurse (APRN) or Certified Physician Assistant (PA-C) to join our dynamic healthcare team. This role focuses heavily on providing comprehensive care to patients with acute and chronic kidney disease, with a specific emphasis on managing dialysis patient care.

The ideal candidate will bridge the gap between specialized Renal/Nephrology care and general Internal Medicine, ensuring our complex patient population receives holistic, high-quality management both in the clinic and during routine care transitions.

Key Responsibilities

  • Specialized Patient Care: Manage an active caseload of nephrology and internal medicine patients, with a primary focus on chronic kidney disease (CKD) and end-stage renal disease (ESRD) patients.
  • Dialysis Management: Oversee, evaluate, and adjust care plans for patients undergoing hemodialysis and peritoneal dialysis. Coordinate closely with dialysis clinics to monitor treatment adequacy, lab results, and vascular access issues.
  • Comprehensive Assessment: Perform detailed physical examinations, order and interpret diagnostic tests (including laboratory panels, imaging, and renal function markers), and formulate precise treatment protocols.
  • Chronic Disease Management: Address comorbid internal medicine conditions common in renal patients, including hypertension, diabetes, anemia, and bone/mineral metabolism disorders.
  • Collaborative Care: Partner with attending Nephrologists, Internal Medicine physicians, dietitians, social workers, and dialysis nursing staff to provide seamless, interdisciplinary care.
  • Patient Education: Counsel patients and their families regarding disease progression, dialysis modalities, dietary restrictions, and medication adherence.


Qualifications & Experience

  • Education: Master’s degree from an accredited Nurse Practitioner program or Physician Assistant program.
  • Licensure: Current, unrestricted state license as an APRN or PA-C. National certification (AANP, ANCC, or NCCPA) required.
  • Prescriptive Authority: Current DEA registration and state prescriptive authority (or eligibility to obtain).
  • Required Experience: Minimum of 1–2 years of hands-on clinical experience explicitly working with dialysis patients or within a Nephrology / Renal specialty.
  • Preferred Experience: Background in Internal Medicine, hospitalist medicine, or managing complex chronic diseases is highly preferred.
  • Skills: Strong clinical judgment, excellent communication skills, and a compassionate approach to managing long-term chronic patient relationships.
