Position Summary
The Home Health Admissions Nurse is responsible for conducting comprehensive in-home assessments to determine patient eligibility and appropriateness for home health services. This role coordinates the admission process, develops initial care plans, collaborates with physicians and interdisciplinary teams, and ensures compliance with regulatory and agency requirements while promoting quality patient care and outcomes.
Essential Responsibilities
- Conduct comprehensive patient assessments in homes, hospitals, skilled nursing facilities, and other referral settings.
- Evaluate patient eligibility and appropriateness for home health services in accordance with Medicare, Medicaid, and payer guidelines.
- Complete Start of Care (SOC) assessments and required clinical documentation accurately and within established timeframes.
- Develop and initiate individualized plans of care in collaboration with physicians and interdisciplinary team members.
- Obtain and verify physician orders for admission and ongoing services.
- Educate patients and caregivers regarding diagnoses, medications, treatment plans, safety measures, and home health services.
- Coordinate care transitions and communicate effectively with referral sources, physicians, case managers, and agency staff.
- Identify patient needs and facilitate referrals for additional services such as therapy, social work, or home health aides.
- Ensure compliance with agency policies, accreditation standards, and federal and state regulations.
- Maintain accurate and timely documentation within the electronic medical record system.
- Participate in quality improvement initiatives and performance improvement activities.
- Serve as a clinical resource for patients, families, and referral partners.
Qualifications
- Current Registered Nurse (RN) license in the applicable state.
- Graduate of an accredited nursing program; BSN preferred.
- Minimum of one to two years of clinical nursing experience required.
- Previous home health, case management, admissions, hospice, or care transition experience preferred.
- Knowledge of Medicare Conditions of Participation and OASIS documentation preferred.
- Strong clinical assessment, communication, and organizational skills.
- Proficiency with electronic medical record (EMR) systems and Microsoft Office applications.
- Valid driver's license, reliable transportation, and current automobile insurance required.
Preferred Skills
- Experience performing OASIS Start of Care assessments.
- Knowledge of home health reimbursement and regulatory requirements.
- Ability to work independently and manage a flexible schedule.
- Excellent customer service and relationship-building skills with referral sources and healthcare providers.
Physical Requirements
- Ability to travel regularly to patient homes and healthcare facilities.
- Ability to sit, stand, bend, lift, and move equipment or supplies up to 25 pounds as needed.
- Ability to use computers, tablets, and mobile devices for documentation and communication.
